Country Manor’s on-site rehabilitation team, Champion Rehab, provides a complete spectrum of inpatient, outpatient, sub-acute rehab and home care therapy services.
Champion Rehab provides outpatient rehabilitation services at Woodcrest. If residents need more comprehensive rehabilitation care, or on-going treatment, they may also utilize our nearly 6,000 square feet of clinical space in Sartell. Our Sartell clinic includes three spacious state-of-the-art gyms, a HydroWorx Warm Water Therapy Pool, private treatment rooms.

Rapid Recovery & Aquatic Center is a short-term rehabilitation facility, with a private entrance and access, located right on Country Manor Campus.
Rapid Recovery features:
- 60 patient suites with private baths, flat screen TV’s, personal refrigerators, and WIFI
- HydroWorx® warm water therapy pool with underwater treadmill
- Occupational, Physical, Aquatic, and Speech therapy, 7 days a week
- 24 hour a day complex, nursing care
- 5-Star Quality Care
- Freestyle dining, including: four internationally themed cuisines, continental breakfast, hospitality cart, and room service
- Full service salon and spa
Due to its unique design, Rapid Recovery is able to provide seamless care for patients through every stage of their rehabilitation journey.
Country Care-A-Van is a Country Manor service, providing appointment-based specialized transportation to residents, tenants and the surrounding community.
Country Care-A-Van has been providing convenient, reliable and trustworthy transportation since 1985.
Our Difference:
- Never be late again! We do not run on a “bus schedule” or “based on availability.” We are an appointment-based service that will pick you up with plenty of time to get you where you need to be on time.
- Door through door service: Our drivers personally pick up clients at their front door and offer assistance as they gather their things before escorting them to the Care-A-Van vehicle. Upon arrival at the desired destination, we ensure that the client has arrived at the intended destination, often helping them appointment check-ins, etc. Our drivers remain with the client until they are certain they are safe, secure and comfortable.
